What to expect at your first appointment?
At your first appointment, you will have some paperwork to complete (that our staff is happy to email or fax to you ahead of time) and we will take copies of your ID card and insurance cards. Our medical assistant will check your vital signs, review your medical history, and the nature of your current problem. Following this, Dr. Harris will complete a physical evaluation and discuss with you treatment options available to try and improve your condition and level of function. Some treatment options are available during the same appointment, while others require scheduling a few days or weeks in the near future. We work with our patients to find treatments plans that help reduce the need for controlled substances while still decreasing your pain and improving your level of function.
What should I bring to my first appointment?
Photo ID and insurance card, any treatment records relating to current complaint including MRI or x-ray reports, list of medications, and your copay (if applicable).
